Your Consultation
Your first visit begins with a comprehensive consultation. During your appointment, we’ll discuss your concerns, review your medical history, evaluate your skin, and answer any questions you may have.
Based on your evaluation, we’ll recommend a personalized treatment plan tailored to your skin, your goals, and your lifestyle.
Some treatments may be performed during your initial visit, while others may be scheduled for a future appointment depending on the procedure and time required.

Your First Visit
To help us provide the best possible care, please:
Arrive a few minutes before your scheduled appointment.
Bring a valid photo ID.
Bring your insurance card, if applicable.
Bring a list of your current medications.
Complete your patient forms before your appointment, if possible.

Insurance, Consultation Fees & Payment Information
Consultations
A cosmetic consultation with Dr. Diane Chiu, Andrea Jones, PA-C, or Olga Govor, Licensed Master Esthetician is $150.
The consultation fee is collected at the time of scheduling and may be applied toward future treatments or skincare product purchases.
Repeat consultations with the same provider regarding the same concern will require an additional consultation fee.

New Medical Patient Visits
The initial office visit for a new medical patient requires a $75 deposit, collected at the time of scheduling.
This deposit reserves your appointment time with your provider and may be applied toward any out-of-pocket expenses associated with your visit, including copays, coinsurance, deductibles, or other patient responsibilities.
If there are no out-of-pocket expenses, the deposit may be refunded upon patient request or applied toward future products or services.

Appointment & Cancellation Policy
We understand that schedules can change. If you need to cancel or reschedule your appointment, we kindly ask that you provide advance notice whenever possible.
Because appointment times are reserved specifically for each patient, cancellations and missed appointments may be subject to a fee.
