Before Your Visit

We're looking forward to seeing you! To help your appointment go as smoothly as possible, please review the information below before your visit.

Appointment Reminders

Five days before your appointment, you will receive a text message or email reminding you of your visit and asking you to complete your patient intake forms electronically before you arrive.

As a courtesy, you will receive a second reminder by text or email two days before your appointment. Your appointment is considered confirmed at the time it is scheduled.

If you need to cancel or reschedule your appointment, we require at least 24 hours' notice to avoid late cancellation or no-show fees and the possible forfeiture of your new patient deposit or cosmetic consultation fee.

What to Bring

Please arrive 10 minutes before your scheduled appointment and bring:

A valid photo ID
Your insurance card (if applicable)

This allows us time to verify your information before your appointment begins.

Getting Here
Please note that ongoing construction throughout downtown Bellevue may affect travel times. We recommend allowing a few extra minutes for possible delays:

We recommend parking as close as possible to the C-West elevator, located at the southwest corner of Building C. Complimentary parking is available in the lot.
